do we need the e111 form ?

You aint in a member state country so your e111 form wont be mutch use!

You do not need the E111 form. If you read the info with the E111 it tells you if you need medical care in Bulgaria (free) you need to provide Passport and NHS card.

But if you have insurance use this you will probably get better care and less fuss.

If you have E111 you can get some recipricol arrangements despite them not yet being in the EU - we will be taking our!
